Cefaclor

Pregnancy: B; Lactation: Use with caution

 

Caps: 250 mg, 500 mg

 

Tabs, Extended Release (ER): 500 mg

 

Powder for Oral Suspension:

125 mg/5 mL

250 mg/5 mL

375 mg/5 mL

Important side effects: Nausea, diarrhea, elevated LFTs.

 

Caution: Significant drug interactions may occur with pimozide (avoid), cyclosporine, HMG-CoA reductase inhibitors. Use with caution in hepatic disease.

 

 

Children ≥1 month:

Usual dose: 20 mg/kg/day (max 1,000 mg/day) in divided doses every 8 hours.

 

Severe or Otitis Media: 40 mg/kg/day (max 1,000 mg/day) in divided doses every

8 or 12 hours.

 

125 mg/5 mL @ 20 mg/kg/day (double for 40 mg/kg/day)

9 kg (20 lb) = 1/2 tsp (2.5 mL) Q8H

18 kg (40 lb) = 1 tsp (5 mL) Q8H

 

250 mg/5 mL @ 20 mg/kg/day (double for 40 mg/kg/day)

18 kg (40 lb) = 1/2 tsp (2.5 mL) Q8H

27 kg (50 lb) = 3/4 tsp Q8H

36 kg (80 lb) = 1 tsp (5 mL) Q8H

 

Child ≥36 kg and Adult:

Capsules: 250-500 mg PO every 8 hours

Extended release tablets: 375-500 mg PO every 12 hours

 

 

Food: Capsules and oral suspension may be taken without regard to food. May take with food to reduce GI upset. Tablets should be taken with food.

 

Important side effects: Nausea, vomiting, diarrhea, rash, elevated LFTs.

 

Caution: Avoid use in patients with history of severe penicillin allergy. Use with caution in patients with history of seizures.

 

Reduce dose in severe renal disease:

CrCl <10 mL/min administer 50% of the usual dose.
