Children ≥3 months to 12 years:

20-30 mg/kg/day in divided doses 2 times daily (maximum 1,000 mg/day).

 

125 mg/5 mL @ 20 mg/kg/day

6 kg (13 lb) = 1/2 tsp (2.5 mL) Q12H

12 kg (26 lb) = 1 tsp (5 mL) Q12H

18 kg (40 lb) = 11/2 tsp (7.5 mL) Q12H

 

125 mg/5 mL @ 30 mg/kg/day

8 kg (18 lb) = 1 tsp (5 mL) Q12H

12.5 kg (27 lb) = 11/2 tsp (7.5 mL) Q12H

17 kg (37 lb) = 2 tsp (10 mL) Q12H

25 kg (55 lb) = 1 tbsp (15 mL) Q12H

33 kg (73 lb) = Use adult dose.

8 kg (18 lb) = 1/2 tsp (2.5 mL) Q12H

16 kg (36 lb) = 1 tsp (5 mL) Q12H

24 kg (54 lb) = 11/2 tsp (7.5 mL) Q12H

≥13 years to Adult: 250-500 mg Q12H.

 

Food: Suspension should be taken with food. Tablets may be taken without regard to food. May administer with food to reduce GI upset.

 

Important side effects: Nausea, vomiting, rash, diarrhea, elevated LFTs.

 

Caution: Avoid use in patients with history of severe penicillin allergy. Use with caution in patients with history of seizures.

 

Reduce dose in renal disease:

CrCl 10-29 mL/min give standard dose every 24 hours

CrCl <10 mL/min give standard dose every 48 hours
