Children 6 months: 15 mg/kg/day (max 1,000 mg/day) in divided doses two times daily.

9 kg (20 lb) = 1/2 tsp (2.5 mL) Q12H

17 kg (37 lb) = 1 tsp (5 mL) Q12H

 

250 mg/5 mL

17 kg (37 lb) = 1/2 tsp (2.5 mL) Q12H

25 kg (55 lb) = 3/4 tsp Q12H

33 kg (73 lb) = 1 tsp (5 mL) Q12H

 

Adult: Usual dose 250-500 mg PO Q12H or extended release 1,000 mg PO once daily.

 

 

Doses of 500 mg Q8H (H. pylori regimens) and 1,000 mg Q12H (atypical mycobacterial infections) have been used.

 

Food: May be taken without regard to food.

 

Important side effects: Diarrhea, nausea, vomiting, headache, taste disturbance, elevated BUN, elevated LFTs, abdominal pain, superinfection, rash, hearing loss.

 

Caution: Drug interactions may be significant, especially: Fluconazole, pimozide (avoid), rifampin, warfarin, benzodiazepines, buspirone, carbamazepine, digoxin, cyclosporine, ergots, disopyramide, tacrolimus, HMG-CoA reductase inhibitors, theophylline, zidovudine, others.

 

Use with caution in persons with liver disease.

Reduce dose in renal disease:

CrCl <30 mL/min reduce dose by 50% or double dosing interval.
