Granules for Oral Solution:

75 mg/5 mL

 

Children:

Mild to moderate infection: 10-15 mg/kg/day (max 1.8 g/day) PO in divided doses three or four times daily.

 

Severe infection: 16-30 mg/kg/day (max 1.8 g/day) in divided doses three or four times daily.

 

75 mg/5 mL @ 10 mg/kg/day divided Q8H

11 kg (25 lb) = 1/2 tsp (2.5 mL) Q8H

22 kg (50 lb) = 1 tsp (5 mL) Q8H

34 kg (74 lb) = 11/2 tsp (7.5 mL) Q8H

45 kg (100 lb) = 2 tsp (10 mL) Q8H

>45 kg = Use adult dose.

 

 

Adult:

Mild to moderate infection: 150-300 mg PO three to four times daily.

 

Severe infection: 450 mg PO four times daily.

 

 

 

Food: May take with or without meals. Take with full glass of water to prevent esophagitis.

 

Important side effects: Diarrhea, may be severe. Nausea, vomiting, stomatitis, rash, jaundice, elevated LFTs, superinfection.

 

May cause severe, potentially fatal, colitis. Encourage patient to report severe, persistent, or bloody diarrhea, abdominal cramping, or blood or mucus in stool.

 

Reduce dose in severe renal or hepatic disease.

 

 

 

Clindamycin Topical

Pregnancy: B; Lactation: Safe

Gel 1%: 30 g

Sol 1%: 30 mL

Lotion 1%: 60 mL

 

Children: Should not be needed until adolescence.

 

Adolescent and Adult: Apply two times daily after cleansing. May see improvement in 2-6 weeks; however, may require 12 weeks for full response. Continue as long as satisfactory response is maintained and significant side effects do not occur.

 

Important side effects: Local irritation and dryness. May rarely cause diarrhea.

 

May cause severe, potentially fatal, colitis. Encourage patient to report severe, persistent, or bloody diarrhea, abdominal cramping, or blood or mucus in stool.

 

Avoid contact with eye(s) and mucous membranes.
