Children ≥6 months to 13 years:

Oropharyngeal candidiasis: 6mg/kg/day on day 1 (max 200 mg), followed by 3 mg/kg/day (max 100 mg) thereafter.

 

Adult: 100-400 mg PO once daily.

 

Food: May be taken without regard to food.

 

Important side effects: Dizziness, headache, rash (may be severe), nausea, vomiting, diarrhea, abdominal pain, elevated LFTs.

 

Caution: May cause elevated LFTs and hepatitis. Advise patient to contact provider if prodromal signs of liver dysfunction occur, i.e., abdominal pain, yellowing of skin or eye(s), unusual tiredness, dark urine, or pale stools.

 

Reduce dose in renal disease. In patients with CrCl≤ 50 mL/min, give usual dose once, then 50% of dose at regular intervals thereafter.

 

Drug interactions may be significant, including: Oral hypoglycemic agents, warfarin, cyclosporine, zidovudine, cisapride, rifabutin, phenytoin, rifampin, benzodiazepines, tacrolimus, hydrochlorothiazide, corticosteroids, buspirone, tricyclic antidepressants, alfentanil, vinca alkaloids.
