Levofloxacin Levaquin® Pregnancy: C; Lactation: Unk Oral Tab: 250, 500, 750mg Oral Sol: 25mg/mL |
≥18 years to Adult: 250-750 mg PO once daily.
Food: May take without regard to food. Do not take within 2 hours of sucralfate, antacids, aluminum, magnesium, calcium, zinc, iron, vitamins, or mineral supplements.
Important side effects: Photosensitivity, dizziness, headache, insomnia, rash (may be severe), seizures, superinfection.
Reduce dose in renal disease: CrCl 20-49 mL/min 750 mg q 48 hours CrCl 10-19 mL/min 750mg initial dose then 500 mg q 48 hrs OR CrCl 20-49 mL/min 500 mg initial dose then 250 mg q 24 hrs CrCl 10-19 mL/min 500 mg initial dose then 250 mg q 48 hours OR CrCl 10-19 mL/min 250 mg q 48 hrs
Drug interactions may be significant, especially: Cyclosporine, warfarin, theophylline
