Ceftriaxone (IM)

(Rocephin®)

Pregnancy: B; Lactation: Unk

 

Vials: 250 mg, 500 mg, 1,000 mg

Vials with 2.1 mL Lidocaine for IM

injection: 500 mg, 1,000 mg

 

 

Children and Adult: 125 mg IM × 1 dose.

 

Children ≥45 kg and adults treat with concomitant azithromycin or doxycycline for presumptive C. trachomatis.

 

Important side effects: Pain at injection site.

 

Azithromycin*

(Zithromax®)

Pregnancy: B; Lactation: Unk

Tabs: 250 mg   

 

Powder for Oral Suspension: 1 g    

 

Children ≥45 kg and Adult: 1 g PO × 1 dose.

 

Use in conjunction with ceftriaxone, ciprofloxacin, or ofloxacin.

 

Food: Oral suspension should be taken 1 hour before or 2 hours after food. Tablets may be taken without regard to food.

 

Important side effects: Nausea, diarrhea, elevated LFTs.

 

 

Ciprofloxacin*

(Cipro®)

Pregnancy: C; Lactation: Unsafe

 

Tabs: 250 mg, 500 mg  

 

≥18 years: 500 mg PO × 1 dose.

 

Treat concomitantly with azithromycin or doxycycline for presumptive C. trachomatis.

 

Food: May take with food if it causes GI upset, but avoid large amounts of dairy products. May take 2 hours before or 6 hours after sucralfate, antacids, aluminum, magnesium, calcium, zinc, iron, vitamins, or mineral supplements.

 

Important side effects: Photosensitivity and dizziness.

 

 

Doxycycline*

(Vibramycin®)

Caps: 50 mg, 100 mg

Tabs: 50 mg, 100 mg

 

Children ≥8 years and Adult: 100 mg PO twice daily for 7 days.

 

Use in conjunction with ceftriaxone, ciprofloxacin, or ofloxacin.

 

Food: May take with food if GI upset occurs. Take 1 hour before or 2 hours after antacids, iron, milk, or other dairy products. Take with full glass of water to prevent esophagitis.

 

Important side effects: Photosensitivity (use sunscreen), esophagitis. May discolor fingernails.

 

 

Ofloxacin*

(Floxin®)

Pregnancy: C; Lactation: Unk

Tabs: 200 mg, 300 mg, 400 mg

 

≥ 18 years: 400 mg PO × 1 dose.

 

Treat concomitantly with azithromycin or doxycycline for presumptive C. trachomatis.

 

Food: May take without regard to food.

Do not take within 2 hours of sucralfate, antacids, aluminum, magnesium, calcium, zinc, iron, vitamins, or mineral supplements.

 

 

 

Important side effects: Nausea, photosensitivity, dizziness.

(Amoxil®)

Caps: 250 mg, 500 mg

 

 

Adolescents and Adult: 500 mg PO three times daily for 7 days.

 

Use in conjunction with ceftriaxone, cefixime, or spectinomycin.

 

Food: May take with or without meals.

 

Important side effects: Diarrhea and nausea. May cause nonallergic maculopapular rash.

 

 

Cefixime

(Suprax®)

May be less effective than ceftriaxone in oropharyngeal infection, but is a useful oral alternative.

Pregnancy: B; Lactation: Unk

 

Tabs: 200 mg, 400 mg  

 

 

Adolescents and Adult: 400 mg PO × 1 dose.

 

Treat concomitantly with erythromycin or amoxicillin for presumptive C. trachomatis.

 

Food: May be taken without regard to food. Food may reduce GI upset.

 

Important side effects: Rash, nausea, diarrhea.

 

Erythromycin Base

(E-Mycin®, Ery-Tab®)

Pregnancy: B; Lactation: Safe

Tabs: 250 mg, 333 mg, 500 mg  

Caps: 250 mg   

 

Adolescents and Adult: 500 mg PO four times daily for 7 days or if GI intolerance, 250 mg PO four times daily for 14 days.

 

Use in conjunction with ceftriaxone, cefixime, or spectinomycin.

 

 

Food: May take with or without meals.

Take with food if causes GI upset.

 

Important side effects: GI upset, hepatitis.
