Oral Suspension:

25 mg/5 mL 60 mL

 

Children 1 month: 5-7 mg/kg/day in divided doses every 6 hours (max 400 mg/day).

 

UTI prophylaxis 1-2 mg/kg/day as single dose (max 100 mg).

 

Adult: 50-100 mg PO every 6 hours.

 

2.5/mg/kg/day

10 kg (22 lb) = 1/4 tsp Q6H

20 kg (44 lb) = 1/2 tsp (2.5 mL) Q6H

30 kg (66 lb) = 1 tsp (5 mL) Q6H

 

Food: Take with food or milk. Maintain diet adequate in protein and vitamin B complex.

 

Important side effects: May discolor urine. Nausea, vomiting, dizziness, headache, rash, hemolytic anemia, granulocytopenia, thrombocytopenia, hepatitis, jaundice, interstitial pneumonitis, fibrosis.

 

Caution: Contraindicated in women at term or in labor. Use with caution in G-6-PD deficiency, anemia, diabetes, or electrolyte abnormalities. Monitor for pulmonary or hepatic reactions or peripheral neuropathy.

 

Do not use in renal impairment. Ineffective if CrCl <40 mL/min.
