Ofloxacin Floxin® Pregnancy: C; Lactation: Unk Tabs: 200 mg 300 mg 400 mg Ophthalmic solution

≥18 years to Adult: 200-400 mg PO every 12 hours.
Food: May take without regard to food. Do not take within 2 hours of sucralfate, antacids, aluminum, magnesium, calcium, zinc, iron, vitamins, or mineral supplements.
Important side effects: Photosensitivity, dizziness, headache, insomnia, rash (may be severe), seizures, superinfection.
Reduce dose in renal disease: CrCl 20-50 mL/min give usual dose Q24H CrCl <20 mL/min give 1/2 usual dose Q24H
Drug interactions may be significant, especially: Cyclosporine, warfarin, theophylline.
