GENITOURINARY - STD, Pelvic Inflammatory Disease (N. gonorrhoeae, C. trachomatis, anaerobes, enteric gram -negative rods, G. vaginalis, Streptococci) General statement: Patients with pelvic inflammatory disease should be carefully evaluated to select those appropriate for outpatient therapy. Patients who do not respond within 72 hours should be reevaluated and considered for parenteral therapy.

Ofloxacin (Floxin®) Pregnancy: C; Lactation: Unk
Tabs: 200 mg, 300 mg, 400 mg

≥18 years: 400 mg twice daily for 14 days. |
Food: Take on empty stomach. Do not take within 2 hours of antacids, magnesium, calcium, zinc, iron, aluminum, sucralfate, vitamins, or mineral supplements.
Important side effects: Nausea, photosensitivity, dizziness.
Reduce dose in renal disease (CrCl <50 mL/min).

Metronidazole (Flagyl®) Generics available Pregnancy: B; Lactation: Unsafe
Tabs: 250 mg, 500 mg *Use in conjunction with ofloxacin. See above.

Adult: 500 mg twice daily for 14 days. |
Food: Administer on empty stomach unless GI upset occurs, then with food.
Important side effects: Dizziness, headache, confusion, seizures, nausea, metallic taste, insomnia, paresthesias. May cause Disulfiram-like reaction - avoid alcohol.

Ceftriaxone (IM) (Rocephin®) Pregnancy: B; Lactation: Unk
Vials: 250 mg, 500 mg, 1,000 mg *Use in conjunction with doxycycline. See below.

Adolescent and Adults: 250 mg IM × 1 dose. |
Important side effects: Pain at injection site. |
Doxycycline (Vibramycin®) Generics available Pregnancy: D; Lactation: Unsafe
Caps: 50 mg, 100 mg Tabs: 100 mg
*Use in conjunction with ceftriaxone. See above. |
Adolescent and Adults: 100 mg PO twice daily for 14 days. |
Food: May take with food if GI upset occurs. Take 1 hour before or 2 hours after antacids, iron, milk, or other dairy products. Take with full glass of water to prevent esophagitis.
Important side effects: Photosensitivity, use sunscreen. Esophagitis. May discolor fingernails.
