GENITOURINARY - STI, Gonorrhea, Uncomplicated Urethritis, Cervicitis, or Proctitis (Neisseria gonorrhoeae) General statement: After neonatal period, sexual abuse is most common cause of gonococcal infection in preadolescent children. Coinfection with C. trachomatis may be common; consider treating for dual infection.

 

 

Ceftriaxone (IM)

(Rocephin®)

Pregnancy: B; Lactation: Unk

 

Vials: 250 mg, 500 mg, 1,000 mg   

 

Children and Adult: 125 mg IM 1 ´ dose.

 

Children ³45 kg and Adults: Treat with concomitant azithromycin or doxycycline for presumptive C. trachomatis.

 

 

Important side effects: Pain at injection site.

 

Cefixime

(Suprax®)

Pregnancy: B; Lactation: Unk

 

Tabs: 200 mg, 400 mg  

Oral Susp: 100 mg/5 mL      

 

Children: 8 mg/kg (max 400 mg) 1´

dose. Coinfection with C. trachomatis may be common; consider treating for dual infection.

 

>50 kg and Adult: 400 mg 1 dose.

 

Children ³45 kg and Adults treat with concomitant azithromycin or doxycycline for presumptive C. trachomatis.

 

 

Food: May administer on empty stomach. May administer with food to

reduce GI distress.

 

Important side effects: Nausea, diarrhea.

 

Ciprofloxacin

(Cipro®)

Pregnancy: C; Lactation: Unsafe

 

Tabs: 250 mg, 500 mg  

 

³18 years: 500 mg 1 dose.

 

Treat with concomitant azithromycin or doxycycline for presumptive C. trachomatis.

 

Food: Take on empty stomach 1 hour before or 2 hours after a meal. May take with food if it causes GI upset, but avoid large amounts of dairy products. May take 2 hours before or 6 hours after sucralfate, antacids, aluminum, magnesium, calcium, zinc, iron, sucralfate, vitamins, or mineral supplements.

 

Important side effects: Photosensitivity, dizziness.

 

 

Ofloxacin

(Floxin®)

Pregnancy: C; Lactation: Unk

Tabs: 200 mg, 300 mg, 400 mg  

 

Adult: 400 mg ´1 dose.

 

Treat with concomitant azithromycin or doxycycline for presumptive C. trachomatis.

 

Food: Take on empty stomach. Do not take within 2 hours of antacids, magnesium, calcium, zinc, iron, aluminum, sucralfate, vitamins, or mineral supplements.

 

 

 

Important side effects: Nausea, photosensitivity, dizziness.

 

 

Azithromycin

(Zithromax®)

Pregnancy: B; Lactation: Unk

 

Tabs: 250 mg

 

Powder for Oral Suspension: 1 g

 

 

Children ³45 kg and Adult: 1 g ´ 1 dose.

 

Use in conjunction with cefixime, ceftriaxone, ciprofloxacin, or ofloxacin.

 

 

Food: 1-g powder packet for oral suspension and tablets may be taken without regard to food.

 

Important side effects: Nausea, diarrhea, elevated LFTs.

 

 

Doxycycline

(Vibramycin®)

Generics available Pregnancy: D; Lactation: Unsafe

 

Caps: 50 mg, 100 mg  

Tabs: 100 mg  

 

 

Children ³8 years and Adult: 100 mg PO twice daily for 7 days.

 

Use in conjunction with cefixime, ceftriaxone, ciprofloxacin, or ofloxacin.

 

Food: May take with food if GI upset occurs. Take 1 hour before or 2 hours after antacids, iron, milk, or other dairy products. Take with full glass of water to prevent esophagitis.

 

Important side effects: Photosensitivity, use sunscreen. Esophagitis. May discolor fingernails.

Treat with concomitant erythromycin for presumptive C. trachomatis.

 

 

Amoxicillin

(Amoxil®)

Caps: 250 mg, 500 mg  

Tabs: 500 mg, 875 mg

 

 

Adolescents and Adult: 500 mg three times daily for 7 days.

 

Use in conjunction with ceftriaxone, cefixime, or spectinomycin.

 

Food: May be taken without regard to food.

 

Important side effects: Diarrhea and nausea. May cause nonallergic maculopapular rash.

 

Cefixime

(Suprax®)

Tabs: 200 mg, 400 mg  

 

Adolescent and Adult: 400 mg ´ 1 dose.

 

Treat with concomitant erythromycin or amoxicillin for presumptive

C. trachomatis.

Erythromycin Base

(E-Mycin®)

Tabs: 250 mg, 500 mg  

Caps: 250 mg

 

 

Adolescents and Adult: 500 mg four times daily × 7 days (recommended) or if gastrointestinal intolerance 250 mg four times daily × 14 days.

 

Use in conjunction with ceftriaxone, cefixime, or spectinomycin.

 

Food: May take with or without meals. Take with food if causes GI upset.

 

Important side effects: GI upset, hepatitis, drug interactions.
