Children: 10 mg/kg (max 500 mg) every night at bedtime for 6 months.
Adult: 500 mg every night at bedtime for 6 months. |
Food: Take 1 hour before or 2 hours after meals. May take with food if GI upset occurs. |
Tabs: 250 mg, 500 mg, 1 g - Caps: 250 mg, 500 mg - Oral Susp: 125 mg/5 mL, 250 mg/5 mL

Children: 1-2 mg/kg/day (max 100 mg) every night at bedtime for 6 months.
1-2 mg/kg/day 10 kg (22 lb) = ½ tsp (2.5 mL) QHS |

Important side effects: Nausea, vomiting, darkening of the urine, headache, hepatitis, peripheral neuropathy. |

Children ≥2 months: 2mg/kg TMP component every night OR 5 mg/kg TMP component twice a week at bedtime for 6 months.
Adult: 1 SS or 1 DS Tablet at bedtime for 6 months. |
Food: May take without regard to food. Encourage fluids.
Important side effects: Photosensitivity, rash.
Reduce dose in renal disease (CrCl <30 mL/min). |
