Valacyclovir Valtrex^® Pregnancy: B; Lactation: Unk Tabs: 500 mg 1,000 mg			Children: Safety and efficacy in children have not been established. Adult: 500 mg PO 2 times daily to 1 g three times daily. Food: May be taken without regard to food. Maintain good hydration. Important side effects: Nausea, headache, diarrhea, dizziness, renal Dysfunction, HUS/TTP. Reduce dose in renal disease: If usual dose is 1 g every 8 hours and: CrCl 30-49 mL/min, then use 1 g every 12 hours CrCl 10-29 mL/min, then use 1 g every 24 hours CrCl <10 mL/min, then use 500 mg every 24 hours If usual dose is 500 mg every 12 hours and: CrCl <30 mL/min, then use 500 mg every 24 hours
