Vancomycin Oral (Vancocin®) Generic Available Pregnancy: C; Lactation: Unsafe
Pulvules: 125 mg, 250 mg
Powder for oral solution: 1 g (provides 250 mg/5 mL), 10 g (provides 500 mg/ 6 mL) |
Children: 40 mg/kg/day (max 2000 mg/day) PO in divided doses every 6 hours.
Adolescent and Adult: 500-2000 mg/day in divided doses PO every 6-8 hours. Usual dose is 125 mg PO every 6 hours.
Important side effects: Oral vancomycin is generally well tolerated and not well absorbed systemically. However, significant absorption may occur in some patients, especially those with active colitis and significant renal dysfunction. |
