SKIN - Herpes Zoster, Varicella (Shingles) (Varicella zoster)

Note: If patient >50 years may benefit from prednisone for pain control.

 

 

Acyclovir

(Zovirax®)

Pregnancy: C; Lactation: Safe

 

Caps: 200 mg

Tabs: 400 mg, 800 mg  

Oral Susp: 200 mg/5 mL   

 

Children 2-16 years: 80 mg/kg/day (max 4 g/day) PO in divided doses four or five times daily for 7-10 days.

 

Adults: 800 mg PO five times daily for 7-10 days.

 

Food: May be taken without regard to food. Encourage fluids.

 

Important side effects: Headache, dizziness, nausea, seizures, bone-marrow suppression, nephrotoxicity.

 

Reduce dose in renal disease

(CrCl <25 mL/min).

 

 

Famciclovir

(Famvir®)

Pregnancy: B; Lactation: Unsafe

 

Tabs: 125 mg, 250 mg, 500 mg    

 

Children: N/A

 

Adult: 500 mg PO every 8 hours for 7 days.

 

Food: May take without regard to food.

 

Important side effects: Nausea, headache, dizziness.

 

Reduce dose in renal disease

(CrCl <60 mL/min).

 

 

Valacyclovir

(Valtrex®)

Pregnancy: B; Lactation: Unk

 

Children: N/A

 

Adult: 1,000 mg PO three times daily for 7 days.

 

Food: May take without regard to food.

 

Important side effects: Nausea, headache, dizziness.

 

Tabs: 500 mg, 1,000 mg     

 

Reduce dose in renal disease

(CrCl <50 mL/min).
